This form is designed to provide the Commission with information required to make an initial evaluation of your complaint.

I claim that the Judge listed below engaged in the misconduct or unethical behavior I have specified below, and I request an investigation of that behavior. This complaint is not the same or substantially similar to a previous complaint from the same complainant against the same judge, I swear under penalty of false swearing that everything I list in this form is true.
